- who your complaint is about
- what you think has been done wrong
- basic personal details
You’ll need to attach any relevant documents as evidence to support your case.
There are four steps to complete the form. You will be unable to save the form if it is incomplete.
It takes about 7 minutes, on average, for people to fill in and submit this form. For some people, it may take longer due to internet connections, how much detail you enter etc.
How will the information I provide be used?
We use the information you provide to help us resolve your complaint. This includes contacting the organisation to notify them of your complaint, discussing the complaint with them, and collecting information about you from them.
We may also share information about your complaint with specific persons or bodies where the information is relevant to the performance of their duties, functions or powers and it is appropriate to do so. If we consider it necessary, we may also disclose information to prevent or lessen the risk of harm to a person’s health, safety or welfare.
If you make a complaint and do not want to be identified to a person or organisation at all, you should request this at the time of your complaint. You can also make a complaint without identifying yourself to us. However, if you are anonymous, we will not be able to get in touch for more details or let you know the outcome.
The office uses third-party vendor EY Sweeney to manage our Complainant Satisfaction Survey . This is a voluntary survey. Your information will only be used with your permission for the purpose of this survey.
